I got a strange glitch once, in Toad Town. I dunno exactly how it happened, but I somehow made the action icons shift their position slightly off from where they usually are, in the corner. When I did that, neither the A or B button did anything. When I left that screen it reverted to normal. I think it happened when I was digging for Mushroom Balls.

Something like that happened to me yesterday. After I dug up a Mushroom Ball, my action commands moved to the middle-right of the screen. After I pressed Start and then exited the Star Menu, my action commands disappeared, but I was still able to use them. When I switched screens, they reverted back to normal as well.
