Well, from what I could see Teller doesn't actually have anything in his hand and he just smacks the back of Penn's hand.
Penn had a block of something (possibly containing the ink/dye and the blade (Perhaps retractable) hidden in the palm of his hand.
When Teller hit Penn's hand it triggered the knife and the ink/dye.
Remember that Penn and Teller are masters at slight of hand ^_^

3.) seperates it
4.) he moves the card box over the right card (blood pack in it)
