I think most Disney tapes had these slides, although it was unusual for them to appear on BBC Video releases, they're usually only kept to Films. Although I don't know anyone who actually owned one of the decoders, I suppose DVD subtitles killed it off.
@Coolhaksta2010 Yeah, on Warner tapes. They were introduced in mid-1993 on Buena Vista tapes. Calafornia Man, from May 1993 (if I'm correct) has the CC screens.
