I may be wrong but I think this song is about the Elephant Man, real name Joseph Merrick. Joseph suffered from deformaties that still baffle medical professionals to this day. His outward appearance was something of a curiosity and so Merrick often wore a hood. His story is one of great sadness because his heart was kind and only a small number of people were able to find that out. Shakira, after seeing David Weir (rangers player), wrote this song about inner (underneath) beauty as a tribute.
