It's actually the expansion that occurs during the reaction that "breaks up" the clog. The liquid carries some of the baking soda into the clog while the initial reaction is occurring. The reaction occurs fully and separates some of the clog. The hot water is then poured down which loosens the binders (i.e. oils, slime, sludge, etc.) by heating them up. You COULD clear a clogged drain with purely hot water, but the expansion of the baking soda and vinegar assists in actually loosening the clog.
