In the third game when Miku gets to choose to follow him or move on... I don't know, it seems like his spirit isn't at rest. He's definitely dead since Rei screams that Miku shouldn't follow the dead if you choose to follow. When all the souls in the third game ending walks to the other side... I like to think Mafuyu is among them.

Ending 3: They escape & leave Kirie alone but Mafuyu's last life shows up & stays with her (see 08:21 )

In fatal mode or Nightmare you don't die with one hit unless the ghosts attack for that attack they perform is strong. In nightmare mode or fatal mode the ghosts attacks are just more enhanced with higher damage but once you get zero function it should be easy for you.

"good" ending is the one with Miku being the only survivor.

there are 2 sacrifices for the gates, so they can stay sealed.

"bad bad" ending is where Mafuyu and Miku leave, and Kirie is reunited with her lover. Kirie is supposed to give up everything to ensure the gate is closed, but her lover was the cause the ritual failed in the first place. WIth her lover there, the gates will fail again.
