The official name of this is Kumikyoku which is part of the Nico Nico Douga Medleys and came out in I think late 2006/2007. I only know five of the popular ones, Kumikyoku, Urakumikyoku, Ryuuseigan, Monogatari, and Nanairo. You can get a full list of medleys at kumikyoku(dot)rintaun(dot)net
