The 'classic' custom bespoke surgical shoe/boot. They can't be bettered for appearance. They compliment the shortened leg by seeming to be a natural (albeit leather) extension and completion of the limb. Beautiful to see.
You are right but they are very expensive to have made and so the reason for the just sponge lift type.These are not any more comfortable as the others.

The build up is 2.5 inches. On some boots I have a 2 inch external build up with a 1/2 inch internal but I prefer the full external better.
