  • just to add. for the benefit of others. RILE--> right sided murmurs are best heard at inspiration and left sided best at expiration. in auscultation always do AR last so that you dont get the patient forwards and back every 2 seconds. also as well as checking lung bases, peripheral pulses,. you should do a quick check for AAA, hepatomegaly, renal bruits.

  • @nobody463 u americans havnt heard of percussing?? u percuss to check for the borders of the heart..in case of hypertrophy,edema etc...

    2)Hands - IE signs

    3)Pulses - RR & RF delay, rate, rhythm, character, collapsing pulse

    4)Face - pallor, cyanosis

    5)Neck - carotid pulse, JVP

    6)Palpate - apex beat, thrills

    7)Auscultate - apex beat & 3 areas, radiation, position LL for mitral and sit forward, at LSE, for AR

    8)Back - lung bases, sacral edema

    9)Legs - edema

    10)Others - peripheral pulses, funduscopy, hepatomegaly, urine dipstick

  • @sadia6655 u can make out the cardiac borders with percussion, incases of cardiomegaly... u can diagnose pul HTN by percussing on 2nf left ICS.....

  • Once I owed a Dr's office a few big bucks and they held me down, then strapped me to the table nude and tickled, taunted and teased me until I agreed to pay in full. I can't remember it was real or a bad dream in the hospital though (due to condition and medication). I think it was partly real though, because I found a feather and mask they were using in my personal belongings at discharge.

  • It would be so much more useful, educational and memorable if she could say why she was doing things. She does it a couple of times but too infrequently...why should he turn on to his left side? ...why listen to the apex of the heart? etc.

  • in some patients, i.e females with large breasts or obese patients, it is difficult to locate PMI, by turning the patient to the left lateral position, the PMI can be located easily. Apex of the heart produces the PMI=Point of maximum impulse.

  • but the point of locating the apex beat is to see if there's cardiomegaly. by turning the patient, there's no significance of locating the apex beat, isn't it?

  • @zaxterry You're abit of an silly arent you! this is a teaching aid! made by a university to aid its students in conducting these examinations properly! the students watching these have examinations where they have to replicate these stages perfectly! if you dont know what you're talking about i suggest you dont say anything because this is a learning tool design to teach FUTURE DOCTORS so they can BECOME REAL DOCTORS!!
