I believe they are caused more by problems caused by a lack of naturally occurring lubricants and /or cartilage. He was mentioning that there were near to 8 millimetres of bone missing. Likely it was slowly ground down through the use of the joint. But as I said, this is only a couple undergraduate courses talking.

It heals because the bone surrounding the knee replacement (part of which was sawed), will grow back and become stronger. Bone constantly breaks itself down, and rebuilds itself throughout a person's life. The rest is just allowing the body to heal itself, which begins at the cellular level.
