its an easy shot - to get the white round just put a bit of top right hand side on the cue ball - then it will zip off each of the three cushions

Part of the problem could be the speed with which you're hitting the shot. Try slowing the hit down a little bit, using a more fluid stroke. By blasting it, you could be putting a little too much top on the cue ball, which could cause it to die coming off the first rail as you've mentioned.

I have a real hard time coming off the third rail for this shot. Even when I hit at near break speed. The cue ball just dies. Any advice?
