It's too bad that you can play this game well through DOSbox, and yet its spiritual sequel, Arx Fatalis, has lots of problems with modern Nvidia cards.
I remember being scared of the spiders (yep, I'm a wimp!) when I played this back in the 90's.
I recommend that you get this game from Ebay...just use dosbox and away you go!
I hereby demand that some game company re-release these games (no remake nesesary..just adjust the audio files config and so on) These games are too good to be forgotten.

What's slightly aggravating in my opinion about this game is the lack of music-driven suspense. While far, far ahead of Doom(yeah sucks to compare, but you know---) in terms of story line, Underworld could still really benefit from some of the dramatic musical pieces that were used in the top-down Ultima's, especially Ultima 7. Theres just a bit too much peace here, walking around looking for a Headless to butcher.
This is one of my favorite games of all time along with Monkey Island II, Star Control 2, and Eye of the Beholder. Come to think of it, I don't think I actually ever BEAT it... I came very close to some room with a faceted gateway thing.. I seem to remember that. I'll have to watch some of the run-through vids that are on here.
Those headless are tough for a little lvl 1 or 2 Avatar. But it gets easier after investing some skill points into attack, defence and a weapon class.
There is a nice bug in the game, though. If you destroy a potion of mana restore with fireballs or bolts of lightning you get some debris that can still restore your mana if you "use" it. And in contrast to the original potion you can use it as often as you wish. Works with any other potion as well.
I sadly wasn't around in this period of gaming (Born 1994) So I mainly played games like EverQuest and what not. But I am going to go back and play things like this for sure. It actually looks better than some of the games I have seen today!

I saw the tune go into the wrong bag, and I thought "You fool!" lol Classic game, loved it. It was so immersive. Although I admit it took me AGES to find the secret passage in my room. I still remember the relief when I finally saw it on the map. :D
i gotta admit, i got stuck at killorn keep and had to check a walkthrough to complete this game. also used the walkthrough to find the madly overpowered flamedeath axe in the vault.
kinda hated how linear it was though the plot's a lot more intriguing than the evil wizard plot in UW1.
Even after all these years and the CRPGs I played afterwards, this is still my favourite one. The eerie feeling brought by the tunes, the magic of being able to move around freely in 3D, the complexity of it all.
Still unbeaten (of course IMO), although Planescape Torment, Deus Ex and Baldur's Gate came pretty close.
I love this game, it was my very first computer game (on an IBM 386 40 Mhz :) It took months to complete it. Il'll never forget "feeling" the Underworld!

I have Exult for the Ultima 7 series as they were not easily run under any Windows system. I remember buying this game brand new for $60 and buying the hint book for another $15. Quite a bit of money 15 years ago.
This game was the best of its time. Dense atmosphere, unique immersion, longevity, graphics and gameplay. Not even Wolf 3D or Doom existed then, picture that. Even a present great medieval RPG as Oblivion doesn't have Ultima's feeling. You had to worry about sleep and eat regularly to survive, not to gain any sort of power. Sometimes finding a loaf of bread could be a true relief...

At the time all this RPG games like UU, EOB, ISHAR, ect. had one thing in common. They were really hard, full of puzzles, traps, though boss's and really really good music.
I should be using DosBox and recording some tricks on this game. Esp. things like the damn tomb level which ALWAYS pissed me off. Or the final battle using Time Stop and also Kill foe spell on the final boss.
Seriously this and System Shock blow everything else out the water, Baldur's Gate, whatever (and I love BG1+2 :D ) - it just goes to show how good Spector, Neurath et al always were.

The best way I've played this was to use my fists for most of the first part of the game. You'd be surprised how powerful they are.

You know, when I first encountered them(um, the headless) in darkness-filled UW1 atmosphere It was really horrible experience... and so was UW2.
