When she eats the heart, i think it's about the heart's surgery of gaga's dad. In the puke part i think she is afraid of self-image, and the fact of one part of her doesn't accept the other part. At the end of the vid, maybe she is afraid of turning into a mannequin or puppet of merchandising. This crazy things make me think..
