It simply just a big blood cloth building up pressure on top of the dura, which gives the patients his neurological symptoms. 1- do not coagulate the scalp with a monopolar (it damage the hair follicles and produce a bad scar
2- the localizatioin was not perfect...
3 there was no need to elevate the temporalis muscle it would be better if he elevated instead a myosseus flap preserving the vascularity and nervous supply of the temporalis and the bld supply to the bone flap, + it will result in better reconstruction latter
4- before the elevation of hitch stiches try gel foam

I thought it was just drilling and sucking the blood. I'm lucky I have no sequels at all except that big question mark scar from the forehead to the back of the ear. @benjie414 It's the haematoma. Nothing more than spilled blood. I think it's the middle meningeal artery that ruptured (but correct me if I'm wrong) - anyways, it is an artery above the 3 protecive brain layers (Above the dura mater, 'epidural' - meaning 'above the dura'), and it is the very cause of the problem this patiënt had.
@The7thSeason , dont watch this if you are not a health care professional belonging to Trauma management. I am watching this to learn about this procedure because after learning this i would be able to perform this maneuver quite confidently and save many lives.
they drill thru the skull? wont it puncture the brain? if they puncture the brain even a small puncture, would the patient die on the spot? this is awsome!!!!!
@GinGeeK Like I already said, the difference is found in the context of the video. Somebody getting a portion of their skull removed during a medical operation to save their life is different than somebody's skull shattering in an accident or act of violence. Same with nudity, watching a woman get a breast exam in order to learn how to do it is different than watching a woman masturbate or get railed from behind by some big black guy
@GinGeeK this vid is in a medical purpose. i served as sergeant in the army as a paramedic. actually i work in a hospital as a care assistant and i am happy to see and marvel how my fellows in the OR work. if you don't have the stomach to watch, don't watch...

Many surgeons confound the terms decompressive craniectomy and surgical decompression because an extra-axial bleeding. I agree it would be enough to fit the flap back in order to avoid true DC complications in this case. Regards.....
