I believe he's referring to a pan handler who use to sing and rotate a musical box (similar function as a jack-in-a-box) while his pet monkey asks people who walk by for change.

@ur69l Actually it's an organ grinder. Pan handler is another word for beggar. Organ grinders were street musicians, more like buskers than beggars.
