A villain is someone amoral, not someone who wants world domination. What makes a villain great is being interesting and entertaining, which is why the Joker is one of the greatest ever.

I can understand your argument about how the joker isn't the number 1 villain. He is definantly not the strongest, but he is definantly more villain-like (Couldn't think of another way of putting it) than any other villain. While other villains have a motive e.g. take oveer the world, greed etc. the joker is just insane. He has no reason for these things which makes him clearly insane, awesome, unpredictable and just plain flawless when it comes to being a villain.
the villains are all psychological, so no they don't just "blow stuff up", they all have a story to them. furthermore, each represents a corrupted philosophy e.g. al ghul's ecological teleological ethics, ivy's environmentalism even dent's obsession with chance
and bane is smart and tough as shown in knightfall and no mans land
and croc is actually more thug-ish as evidenced in arkham asylum when batman calls him an animal
You need to get your hands on the album Batman: Arkham Asylum. Reading that gets you treated to Joker dressed in all the horror and madness he is worthy of. Also, if your idea of a villain is world domination and nothing else, Batman really isn't your thing.

I don't think you're getting the point of the villains of Batman; he's not the kind of hero that takes on world conquerers, he takes care of the normal thugs that the police are supposed to capture. That's his shtick; crime took his family, so he's fighting back against crime, and crime doesn't mean taking over the world. He's protecting his home town from the bad guys that would threaten it with robbery, muder, arson, etc., and these guys are the most dangerous criminals of them all.
