I'm going in for a laproscopy to check for endometriosis December 22nd. endometriosis runs in my family, 80% of the females in my family suffer from it. I am hoping that this surgery can give me some clues as to why I am in such pain. its comforting to see the procedure in this video.
In general Co2 gas is used to "inflate" the belly that is why we have this excellent view. One problem is that if too high of a gas pressure and flowrate is used the pt absorbs a lot of it and becomes acidemic. Then anesthesia gets upset and one has to dial it back. If the rate is too low one is slowed down because suction rapidly deflates and ruins the view. electro-Cautery causes a lot more smoke and need for suction ("foggy") than laser. None of this presents big risks or problems.
0.19: Here is the money. The uterus is in the air because during the surgey a device called an intrauterine manipulator is inserted via the vagina. Its simply a stiff little rod that allows us to move the uterus around. thickness roughly of a straw.
The alien like "goo strings" underneath the uterus are not physiological. the white stuff on the uterus and they themselves are adhesions.
You can see the broad ligaments to the side of the uterus. With imagination also one round ligament.Fat=yellow
0.03s: This view was done for us. The surgeon used a 3 pot setup. umbilical port has the camera. From your viewpoint left port is suction and lavage (red and blue button action self explanatory) Right is obvious.
0.05s: surgeon pushed down large Bowel. If you pick a steep trendelenburg oftentimes its not even in your way. The heavier the patient the less often I seem to be lucky there...The pink thing emerging behind the bowel is the uterus.

33 yr old. Just been diagnosed with Endo, due a laparoscomy soon to laser cysts and check to see how bad the Endo is. My Gynacologist says there's a good chance I might need a Hysterectomy. Had problems with my pelvis since I gave birth to my Son, and had SPD too. Seemed to disappear when I had my daughter, but is now back again. My womb has 'moved' and is in the wrong place apparently. I think I'll opt for the Hysterectomy if asked. You live your life with pain and get used to it I think.

Here’s what to look for: the first thing we see is the metal medical “arm” squishing down tissue so the female parts are exposed. Then it looks like a bulbous jellyfish with tentacles hanging down. What I’m calling tentacles are endometriosis lesions. Look for the bluish tent to some of the tissue. That’s endometriosis. The abnormal tissue has wrapped up her organs and has fused tissue together that shouldn’t be fused together. I'm sure this woman hurts.

this uterus has significant adhesions, there is some intraabdominal fat. (not an unusual amount)
