con'd So if you found that these JV pulsation coincides with the carotids (call it systolic expansions) then it is due prominance of the v wave and hence the diagnosis is TR. (note: Carotid pulsations are medial to the muscles not lat to it)
Con'd I want to remind you all that in case of doubt you simply try obstruct the flow by puuting your hand at the base of the patients neck, if the pulsations decrease then it is venous as carotid pulse needs much pressure to be diminished. Then it is important to time the JVP with carotid pulse on the other side. a wave is just before the carotid upstroke and the v wave is just after it.
Definetely Aortic Insuficiency. The typical "carotidal dancing" sign(in spanish). It would be great to have more videos like this one, especialy for those preparing for the step 2 and CS...
