@nunoyobiznis It was probably a modded server where either the axe had more power or the Witch had more hit points. On Easy, the Witch has 500 hit points, and the axe only deals 70 damage. There may, however, be (or HAD been at the time that that happened) some sort of scripted thing where if you hit her in a specific spot with a sharp weapon it would crown her. Not sure, though.

I've noticed the same thing. A Charger was pummeling me while Coach was standing right next to him. After 3 long seconds of thinking about chocolate helicopters, he finally shoots the Charger, pauses, then shoots again. L4D2 CPUs are too busy thinking about stories of Keith, chocolate helicopters, hating things with Francis, and constantly thinking of ways to stand by Rochelle in each of the campaign posters.
