I placed the original audio in helllabs org, subdirectory media.

It's called "Distant Call" and it's originally a Protracker mod file written by Mr.Man of Andromeda. You can find this file googling for "Distant Call" "Mr.Man".
I was waiting for someone to make this comment. There are two reasons why the effect isn't there. The first, and weakest, is that their placement make them rather unnatural to play (but for accuracy they could have been played). The real reason is that I had to add reverb and envelope release to compensate the lack of a sustain pedal, and under that circumstances the extra note sounds like a chord instead of a slide.
I just bought I juno g and I am so happy with what it can do. I think the Juno G makes a great first synth because the price is very affordable and it can do so much that you are forced to learn all the parameters in and out to fully take advantage of what the Juno G can do. I've just scratched the surface. Check out so loops that I created using the Juno. Man am I having fun!!
A custom patch based on Nice Piano from the XV Collection, as stated in the description. Get Nice Piano, increase the cutoff frequency and envelope release, and transpose down one octave.

Nobody does as far as I know. It's instrumental only and written as a 4-channel Amiga Protracker module file.
