Yet Reznor was a heroin addict and think the "the needle tears a hole" is pretty obvious. So it references addiction as part of the pattern of depression. I have also seen references to Reznor cutting himself, so I think that may be an aspect as well.

@ianmason2003 Yes on both accounts. He sings the song from his heart, making it his own. The words reach to me regardless of where they originate, but the power and emotion he brings really makes me think on my whole existence and how awesome life is.
