Marcus Kane was an ice-cream truck driver with split-personality disorder. His alter-ego is Needles Kane. Needles "took over" Marcus's body, murdered his family, and became a serial killer. (See Twisted Metal 2012).
Marcus does regain control from time to time, but he is no longer the dominant personality. Whenever he is in control, it feels like he's in a dream (most likely because he is only in control when the Needles personality is "sleeping").

It's hard to connect, though most stories do have to do with Sweet Tooth. TM:Black for example, had Sweet Tooth as a pure killer. No mention of Marcus in that game.

I already beat the game with Minion and I figured out the codes myself. I know it was all in ST's head, however, the stories do not connect with the others.
The story from 2012 TM and any of the others do not connect.
He wanted the nightmare to end. In a person's dream most of the time they refer to themselves by their true name. Needles was his true name. Hence Sweet Tooth was who he truly was when he woke up and the persona of marcus kane only existed in his "nightmare."
I'm thinking Sweet Tooth is more the nightmare split personality, this would explain the overly average/generic look they gave Marcus, which I think is specifically meant to contrast heavily with Needles.
Nightmare, Sweet tooth sounds a bit like James McCafferey.... *GASP* MAX PAYNE REFERENCE!!!!!!!!!!!!

What REALLY doesnt make sense is the hair. He can't grow it and lose it between personality changes.
