i can't believe i get it. at least i think i do. the last scence of the movie gave it away. he told himself to look for john G. who was teddy. teddy wasn't even the killer but he made his own self believe that he was the killer. so therefore sammy jenkis was not real, he just made himself believe that he was real. he was really sammy jenkis. he made himself go on a wild goose chase to make himself find the killer of his wife. he didn't want to remember the truth. he was the one who killed her.

Remember when Teddy told him that it was just some guy? That wasn't good enough for Lenny, so he made a puzzle he couldnt solve.
