I kind think that all of the nasty red stuff with the wires is shepards insides, because in me2 when he fell down to the planet and died, shortly after, Jacob tells him that he's still himself, just with some new pieces is all. The rest look like protheans, the citadel in a defensive form, and reapers or collectors.

I'd guess that the "Old Machines" were created billions of years ago by some race with the purpose to win a war or just wanted to rule the universe, and ofcourse it backfired and killed their creators, somewhat similar like what happend with quarians when the Geth became aware and started the Morning War

Actually, biotics are only new in humans. The asari have had them for alot longer. The Reapers don't care that we have biotics, they are strong enough. The Reapers don't have a reason. They just do this because it is a progressing cycle that has been going on for a billion years. They don't have a reason for this mass extinction, they simply do it. Biotics has nothing to do with it.
