Yang qi is manifested to the skin through powerful exhalation. Screaming is not necessary, but it does mean that he is exhaling a lot. You generate qi in your lower dantien by relaxing and becoming more yin, and release it through muscle contractions and strong exhalations turning it into yang. Or at least this is my humble understanding of the topic.

Actually there IS a bone in the throat. It's called the Hyoid bone.

The hyoid bone helps to support the tongue and elevate the larynx when you talk or swallow. It's the only bone of the body that does not articulate with any other bone. The hyoid is suspended above the larynx and is anchored by ligaments to bones in the skull.

This is a trick practiced by vagabond martial artist for centuries. Notice how he bends the knife towards his chin. The pressure is not going straight in but upward, by the time it could penetrate, the point is resting flat on the neck. My buddy is a shock magician, he has no martial skill what so ever and he does this with a machete and a 12' knife. Look up mind and body kick ass moves. It's not that great. I know plenty of people who can, and often do (as a party trick) snap chopsticks on their necks like that. And if you search around Youtube, you will find videos of Chinese masters who put whole swords to their throats, and then have a chat whilst they do it. It's not qi. It's just a trick. Almost anyone can do, if they put their mind to it (don't go off and try it, though!).
