Those are his real parents. His show is long gone and he still has recent home videos of visiting his parents. As for the dad is sick skit, they played along with a lot of stuff. I remember Tom woke them up with a fire extinguisher and they were hating him the next day. After the clip was over, his parents stepped onto the stage with fire extinguishers and sprayed him for revenge. They were good sports with the insane stuff Tom did.
