3. "Maxed 1 def" means maxed in every stat except Defence, which is 1. "99 attack / 99 strength / 1 defence / 99 ranged / 52 prayer / 99 magic / 99 hitpoints" is not maxed 1 def.

4. Consider using the best equipment available to you, or close to it.
