That's horrific, it still amazes me to this day how little people know about health and safety, or should I say how little training people are given by there employer.

Thousands of people die every year because of people being unaware of how dangerous certain situations are that we take for granted.

The person who opened the door should of made everyone working in that area aware that there was a hazard and made the area safe by erecting a barrier with signs.
