If you want the theme, it's about self sacrifice. "Avert the children," at around 2:26 explain to not look back to see the blood. I believe the message 'move on and never look back' are also present in this song. I feel it's one of QOTSA's most powerful songs, despite the very few verses Josh sings.

The word ''blood'' is on both times followed by ''river in the road'', so it was maybe meant like ''bloodriver (river of blood) in the road''. The message could be that you should never look back at horrible things (like river of blood) you have done in the past, instead you should just focus on the future. But that's my opinion.
