I'm sure others have asked this question: but who wrote this song? I know Alan Moore is the singer but who wrote the original lyrics, what album was this from, and what band and/or company made it?

I reason why my question is so detailed is because I hope to use this song as a source for my senior thesis. So I would be grateful for the answers as soon as possible.
