She's shown sleeping with it and caring for it because obviously, when you have a loved one dying of cancer, you try not to be upset infront of them; instead you comfort them. The picture of the 'thing' in her car represents the fact that it's always on her mind. The old woman doesn't take 'it' to hospital, because it is her. Notice how she doesn't look at the old woman after the 'thing' has died.
