Hope this helps: I have had my cartilage pierced for years, and it barely hurt at all when I had it done....in fact, it hurt much less than the other parts of my ear. It did take a bit longer to totally heal, but nothing really awful.

His earring is called an "industrial" or "scaffolding". Different people call it different names like a lot of the more uncommon ear piercings. IMO cartiledge ear piercings hurt more than any other body piercing, but some people will tell you the opposite. It really depends on your pain tolerance. If you were to get it done, I'd recommend getting 2 separate rings to let them heal, THEN when they are, put in the proper longer bar. The reason why is because they rip & get infected easier together
