Guys, If this don´t work is because you PS3 controller wasn´t recognized by your console. Try using a dualshock and try the same steps. Then use the option 5 of menu (Restore PS3 System) and then follow the instructions and later try to system update procedure from a usb drive. For me that works and then restored my data.
guys this might not work bcuz my friend opened my ps3 when that happenend to me and there were a couple of cords lose and he just plugged them back in so u might want to look inside just in case and if it works

I just insalled a new hard drive and got this message. I tries this his method without success. Anyway, for those who still get this message heres what i did. Your gna need a usb stick, now download the latest ps3 update via pc/mac. on the usb stick make 2 folders, name first 1 PS3, go into PS3 folder and make another named UPDATE. Make sure u do it in CAPS. After download place it in folder UPDATE. now get the menu screen up like in this vid on ps3. Place usb stick in ps3 choose update ps3.

Guys once your done restoring you ps3 and than it says that the hard disk failed and has to be restored just click ok and as soon as it hits 0% take out the HDD and than something will pop up and than u put it back in and than click Yes and than Yes. and that should do.
i think the best option to choose is number 5 because it will format your ps3 hard drive. it that doesn't work then try 4. still doesn't work then you should replace the hard drive, maybe your hard drive is broken.
i have a external hard drive connected with sata and esata and it worked untill now when i unplugged it and then plugged it back on...but when i restore the file stuff... will it delete everything?

solution to those who cant access the recovery menu:
i cant access it to, and its probably because i just upgrade my hard dribe to 500gb...what i do is i switch back to my old 80gb hard drive that comes with my ps3, then when i do the 2 beeps thing it worked, i'm in the recovery menu, so i chose the 3rd option like OP did then i could access the xmb like normal again ,
then i switch back to new hdd n i didnt even need to do recovery again...my ps3 turn on like normal n i could access xmb..

@snipshow777 i will sub if you can help, it sterts off at the bit restore hard disc. then when its done it restarts itself and asks again, so i went on the cool menu thingy. (lol) and i pressed restore hard drive. but it said it could noot find it or something along those lines.
@FreeKicksPFC try "restore file system" first and if that doesn't work try "restore ps3 system". That second one MIGHT delete your trophies but im not too sure, but it will definitely delete your game saves.
I have a question, I have my 640 gb hdd from my old ps3 and I want to put it into my new ps3 slim, my 640gb has all my movies, save game files etc on it. when i tried it earlier the ps3 slim stated that the 3.60 firmware was not available press start & select to search for firmware......My question is, is it possible to place the hdd with an older version of firmware on my new ps3 slim. So is this supposed to be a *permanent* fix? Because I get this error maybe once or twice a month and turning the system off from the back, waiting a few seconds, flipping the switch back on and turning the system on works just fine. I'm not using the original HDD but does that matter? It seems like this is way more sketchy unless it actually fixes the issue.
@omg5000 depends, sometimes its permanent other times you might have to repeat it, when it happened to me, i would just repeat it and it went away and fixed itself i guess over a few months. Sketchy? this is exactly what sony told me to do. Call them. see for yourself. Using the original HDD shouldn't matter
@snipshow777 Oh I am not doubting that this is what Sony told you to do. By sketchy I meant going to some unknown BIOS looking screen and dealing with options like "Restore File System" and "Hard disc corrupted" and all that is a lot more sketchy to me than turning the system off from the back, waiting a few seconds, and turning it back on. Just saying, kudos to everyone that it worked for though.

i think the best option to choose is number 5 because it will format your ps3 hard drive. it that doesn't work then try 4. still doesn't work then you should replace the hard drive, maybe your hard drive is broken.

i have a external hard drive connected with sata and esata and it worked untill now when i unplugged it and then plugged it back on...but when i restore the file stuff... will it delete everything?

Nearly no one can access this freaking safe mode nowadays if it says "Cannot start. Correct Hard Disk not found.". :S

sometimes this menu helps, although mine is fucked up so bad i cant get the menu to come up becuz my ps3 fell on the floor and im pretty sure the HD broke, so fuck. another 100 bucks to sony for the win!

solution to those who cant access the recovery menu:
i cant access it to, and its probably because i just upgrade my hard dribe to 500gb...what i do is i switch back to my old 80gb hard drive that comes with my ps3, then when i do the 2 beeps thing it worked, i'm in the recovery menu, so i chose the 3rd option like OP did then i could access the xmb like normal again ,
then i switch back to new hdd n i didnt even need to do recovery again...my ps3 turn on like normal n i could access xmb..

@snipshow777 i will sub if you can help, it sterts off at the bit restore hard disc. then when its done it restarts itself and asks again, so i went on the cool menu thingy. (lol) and i pressed restore hard drive. but it said it could noot find it or something along those lines.

@FreeKicksPFC try "restore file system" first and if that doesn't work try "restore ps3 system". That second one MIGHT delete your trophies but im not too sure, but it will definitely delete your game saves.

I have a question, I have my 640 gb hdd from my old ps3 and I want to put it into my new ps3 slim, my 640gb has all my movies, save game files etc on it. when i tried it earlier the ps3 slim stated that the 3.60 firmware was not available press start & select to search for firmware......My question is, is it possible to place the hdd with an older version of firmware on my new ps3 slim. The older firmware is 3.50?? cool vid by the way :-D

@MOOOOOOOOOOOOOONNNNN Possibly, try searching for that on the sony website, also, since PSN is down that could be why the 3.60 was unavailable. Try waiting until PSN goes back up.

So is this supposed to be a *permanent* fix? Because I get this error maybe once or twice a month and turning the system off from the back, waiting a few seconds, flipping the switch back on and turning the system on works just fine. I'm not using the original HDD but does that matter? It seems like this is way more sketchy unless it actually fixes the issue.

@omg5000 depends, sometimes its permanent other times you might have to repeat it, when it happened to me, i would just repeat it and it went away and fixed itself i guess over a few months. Sketchy? this is exactly what sony told me to do. Call them. see for yourself. Using the original HDD shouldn't matter

@snipshow777 Oh I am not doubting that this is what Sony told you to do. By sketchy I meant going to some unknown BIOS looking screen and dealing with options like "Restore File System" and "Hard disc corrupted" and all that is a lot more sketchy to me than turning the system off from the back, waiting a few seconds, and turning it back on. Just saying, kudos to everyone that it worked for though.
