What's the benefit of having the patient awake during the surgery? Is it just a safer anesthetical practice? I've witnessed about a dozen regular CABGs and beating heart ones under regular anesthesia, but this is a 1st for me.
using regional anaesthesia avoids general anarsthesia, endotracheal intubation and all problems related to either. It,s just a fast tracking tool. reduces ionotropic requirement, & its sympatholytic effect stabilizes heart rate as well as blood pressure during procedure.
So you didn't stop the heart? Is stopping the heart an old technique that you don't need to do anymore? I noticed there was that thing connected to the heart that kept that specific portion of the heart from moving while you work on it. That's pretty cool.

I work with these pts alot and I was wondering when the heart is beating is there a greater chance of arrythmias such as vtach vfib or the occasional pvcs versus the traditional technique... Or do the meds given aid in the relaxation of the cardiac muscles.... Very interesting video
pvc's are there but if you place the intracoronary shunt quickly it allows distal perfusion & since there is no ischaemia therefore no major arrythmias.

well freind, it depends on how many grafts are to be done, what areconduits planned, which are vessles to be grafted. whether onpump or offpump, what is pt's lvef.usually if you plan awake surgery IMA harvesting is to be done extra pleuraly sparing pleura which is time consuming. usually a four graft case requires 6 hrs including anaesthetist time.
We have uploaded videos on aortic & mitral valve replacement with chordee preservation as well as total arterial off pump awake surgeries. you can go through. thanks
i need to have a valve replaced as it is to narrow and leaks, i also have a 2nd degree blockage . just worried on the recovering pain and the chances of any mistakes occuring during the operation going to get it proberbly within the next 6 months
which valve needs replacement? 2nd degree block means AV block? i suppose. usually with thoracic epidural anaesthesia there is no pain perceived in post operative period . thanks

That's what beating heart surgery is all about. The instrument that stabilize target area is called octopus.

how long the operation takes place? here in our hosp. using the hear-lung machine the least is 4 hours.
