That happened to me in one of my sims birthdays, luckily he got down himself and I didnt have to surgically remove him from his mother.

Sometimes my sims have a birthday and they don't grow up, so they have to blow out the cake again.

Similar thing happened to me, one of my sims made drinks at a party then when I told him to do something else, the platter of drinks just stayed on his hand o.O it was there for about a week but I think I finally got rid of it by editing his outfit :p not so sure though :)
