Actually, while I know it is in reality as crazy as all hell, to the good Doctor here, the cutting himself is a way to keep himself grounded in (what passes for it to him, anyway) reality; the pain helps keep him grounded in the here-and-now while preventing him from slipping any further into his psychosis. Only problem is, nearing the end of this movie, not even the pain will help him keep his grip on what precious little remains of his sanity.
