The boiler explosion was definitely caused by Alessa, but maybe it wasn't necessarily the fear and stress from the ritual, but rather Alessa herself trying to find someway to prematurely end the ritual before it could be carried out, attempting to immolate both herself and the ritual's starters as a way to try and prevent the demon from entering our world. However, her attempt failed, and she split her soul as her very last prevention method.

Alessa spreads the seal of Metraton so she can spread her influence, but the monsters she creates aren't evil, they're tools she is using to protect herself from the evil Dahlia..
When Harry fights past the monsters Alessa creates a character based on Lisa to slow him down. That doesn't work either and Alessa gets captured.
Alessa is a scared child trying to hide, so she creates monsters from her own childish fears. (dogs ect.)

The European release censored the game by taking out the monsters in the school which look like children (key word being children). They have different standards in the UK, which is why the game Rule of Rose was banned there (also a game revolving around violence toward children).
Therefore, the note was likely added in for the European release as a way to censor the violence towards the child Alessa.
Rule of Rose is about a young woman reliving and coming to terms with the tragic events of her childhood. There is bullying in the game both physical and otherwise, Jennifer's childhood self attacks Wendy after she has Brown (Jennifer's dog/puppy) killed, and near the end of the game it's heavily implied that everyone in the orphanage except Jennifer was murdered, but that is NOT what the game revolves around.
@Zorkatron86 They're probably the little voice-overs at the ends of some of Silent Hill 3's songs. I don't remember hearing some of these either, but it's doubtful that they hired her.
