...you realize that's the first time they've ever shown the entire outside of the hospital?...I don't thinkl it's real though...the train that went by is real, but the actual outside of the hospital was a simulated image or something.

Watch the show seasons 1-3, they used real hospitals in Chicago on location to fill in for the entrance, and sometimes there was no train. After that, they built a set for the ambulance bay for the rest of the series in LA.
