I believe the swan head is a reference to "Koihime Musuo". I can't say 100%, but that's were I saw it first. However don't watch it if you don't like mature content (as in 17+ rating).

@steeljawX Actually, it's used rather frequently. Most recent I've seen is in Higurashi no Naku Koro Ni. What struck me as odd in this, though, is that it's usually the loser who has to wear that thing, unless Kohime Musuo uses it differently. It's been many years since I saw that Hentai, so I'm not sure anymore.
