He's a guy. He said as much on his site. It's not uncommon for guys to cosplay girls, or girls to cosplay guys. It's called crossplay, and there is absolutely nothing wrong with it. Most of the guys who crossplay are perfectly straight (or bi...lol), and only do it when cosplaying at conventions. Or, as this guy is doing, playing music for the whole world to hear.

Actually, Tissue-hime did say he was a guy. It's not that uncommon for guys to dress as girls in the Japanese music industry, nor is it something taboo. Most of the Japanese population do not care if a guy cross-dressers so long as he looks pretty. They have entire game shows based around it, and special maid cafes where all the maids are crossdressing males. So yeah.
