The guy and her are really into each other, and one night he has to go out but doesn't want to wake her up to tell her where he's going, so he puts a note in the pocket of the jacket she always wears. He then gets killed, but she doesn't know that. All she knows about is the note.
(con't) The guy never comes back (of course), so she begins thinking that the note was really a "dear john" letter, but it's not! LOL! That second verse is kind of "weird" on purpose, because she is weird. She spends almost every non-sleeping minute, thinking to herself in a very Dr. Jeckyll & Mr. Hyde-like manner :-) The truth is that she doesn't actually go anywhere physically, and no one actually taps her on her arm. She's going to these "places where we used to go" inside her head.
(con't) The "someone tapping her on her arm" and "asking her if she'll be okay", is actually HERSELF (Dr. Jeckyll) doing/saying that to HERSELF Mr. Hyde). The last line ("The pressure's too much, gotta get out of the room") is how she gets herself to snap back into the real world (i.e. stop thinking to herself). "Room", in that context, is a metaphor for her thoughts/mind. So you see, the verses are really a reflection of what she's going through mentally.
