I'm not certain what happened there, but then again, i've never died from a Massive Sanity Failure in the game. With all the morphine you were taking, I was expecting you to OD on it. I don't know how much of the morphine that would take, though.

I think that's what happens when he commits suicide. Just if one was insane, then they may be looking at their hands as a sort of reality check.

He always either commits suicide (If you're holding a gun?) or loses sanity.
