How Not To Setup A Catherine Wheel

When you set up a catherine wheel,you ensure it can spin freely(but don't knock it into the post too much!) but also check the nail isn't too loose for it to fly off either. That bang must have been a faulty wheel jet.

the problem was you nailed it in too far. its a common mistake i made myself.
