He actually moved the butchers knife Dead used to cut himself into the frame, which is why it's on top of the shotgun. The picture leaked somehow, and was used as the cover for a bootleg recording of a live show.

He had a metal records shop in Oslo called Helvete (Hell). The bootlegs just took already released material, or some unreleased rehearsal tapes and released them. To do that, you'd have to be close to the band anyway.
