He's not sure whether they are real, because, now that he is on his meds, he can't see them. But Christians speak of an "anointing" which makes certain people different (able to see things that others can't see). He also sees wind when he looks at trees, and wonders if this has to do with the fact that plants produce the oxygen we breathe.

Mental health is relative, so yes, it is because they are so different that they need to go through therapy in order to be more functional members of society. He said everyone can't see that and he can't tell whats real or not real even though it's what he sees and that's what makes everyone different. Then he compared how people are different to everything else in life, like nature, is different.
