I'm going to assume you were using a tank spec, and there's only one thing I can suggest. I prefer using glyph of Disease. This allows me to use death strike instead of spending those runes / gcds on diseases.
hard to say what went wrong given your description. it's important to keep yourself topped off whenever possible. In this video I rarely ever drop below 70%.

i used death strike every chance i had and id get damage harder then i heal and i heal for 4800 a death strike
