they never explained how the duchess returned, but since Hatter is alive as well in madness returns (though in pieces and you end up helping Hatter get his body parts back lol)... and he is assumed "dead" again at the end of chapter 1 just to re-appear right before the final battle on the infernal train.. we can assume that the denizens of Wonderland can never die, in the end, they're just her imagination.
