The best thing to do is to trigger Dr. Salvador, then go back to the house, wait for him to swing then knife his head. Afterwards, knife him till he gets up. Rinse and repeat.

Two Tricks: Going back through the door at the begining nets you an extra scene. HEATHEN, HOW DARE YOU INSULT THE INCENIARY GREANADE! Seriously though, the flamer does have it's uses. They just aren't aways immediately apparent.

When Dr.Salvador was born, he was accidently dropped on his head 200 times because the doctor that was holding him used numbing solution on his hands, and on the 200th time a knife popped up and it went through his head, but he survived. Everyone in his school made fun of him so he surgically put a rotten potatoe/onion bag over his face and moved to a village Las Plagas in his excavation in the cave with 100 billion other people and now he's treated like a Ganado king.
