I have studied this subjec of SIDs for over 15 years. A causative agent may be pertussis, since a child is only protected (from Pertussis) a month after his second dose (5m/age). Pertussis may seem like a cold with no fever. In my high risk population no child has died from SIDS. Women should be vaccinated against Pertussis prenatally. If the mother produces IgG, the newborn should also be protected postnatally. A complex issue, you're right.
Perhaps the number of SIDS death has decreased because we have become better at diagnosing. SIDS is just another term for "I have no clue"
