Disabled toilets can be accessed from the outside with a key. I've used them at aiports in the past as a changing room, privacy etc. I wouldn't ever dream of walking out of one with my feet inverted and my tongue sticking out.

I was using a normal, non-disabled stall in a train station bathroom once and I wasn't sure which thing was the flusher. There was a button on the wall so I pushed it. When nothing happened I realized it was the emergency call button!
